We have bluetooth enabled devices set up designed to enable doctors, health plans, and caregivers the ability to monitor daily vital signs of illnesses such as Diabetes, CHF, and Hypertension.

Patient Care Is Our Priority

We Have A System That Works

1. Once we have checked the eligibility of the patient, we contact them and set up a time to visit and personally drop off our monitors.

2. Our telemonitoring specialist takes proper safety precautions while at client's homes and teaches them how to utilize our user friendly monitor.

3. Once the patient is satisfied and comfortable using the monitor, the telemonitoring specialist checks on their vitals frequently from her device.

4. Vitals are sent to the physician as they are received, with weekly reports from our telemonitoring specialist and continued daily communication with our patients.
