SCOPE AND FOCUS:

The scope and focus of the home health service to patients are primary when home medical attention is required / or requested to do the following:

  • Improve the patient's condition so as they are able to get around as soon as possible
  • Meet the patient's activities of daily living.
  • Meet any of the patient's medical emergencies during time of care.
  • Provide the patients with a measure of time that would no longer need home care and provide information for after care.

  • Types of services and equipment required
  • Frequency of visits
  • Prognosis
  • Rehabilitation potential
  • Functional limitations
  • Activities permitted
  • Nutritional requirements
  • Medications and treatment
  • Any safety measures to protect against injury
  • Instructions for timely discharge or referral
  • And any other appropriate care that is referred by attending physician
  • Process, document, and keep any and all required patient's clinical reports and forms for state and federal agencies or physicians.

WILL INSURANCE COVER THESE SERVICES?:

Our services will be covered by your Medicare benefits.  We also accept private insurance.  (BC/BS; WellCare, Care Improvement and Care Improvement Plus) Our agency will do all the pre-authorization and arrangements on your behalf according to your primary physician's specifications.


 
  • WHAT ARE THE REQUIRMENTS NEEDED FOR A PATIENT TO BE ELIGIBLE FOR SERVICE?:
  • A patient must meet one or more of the following:

              Reside in Service Area:  Cameron, Hidalgo & Starr Counties

  • You are homebound

     

  • Your primary care physician has ordered our services after a face to face encounter. 

     

  • You require skilled nursing, home health aide, physical, occupational and speech therapy services.
  • Referrals are accepted by either telephone, fax or original physicians' signed orders
